Polk County Music Festival & Cancer Benefit

Each year, Sheryl’s BreastFest selects one local individual or family facing cancer to be

our honored beneficiary. This person becomes the heart of our annual music festival and fundraiser, receiving both financial support and the love of our community.

 

We invite you to submit a nomination for someone who is currently undergoing cancer treatment

and resides in Polk County, Florida.

Opens: January 1, 2026
Closes: March 1, 2026 at 11:59 PM

•  Applications submitted before January 1, 2026 will not be reviewed or considered.
•  Applications submitted after March 1, 2026 will also not be accepted.

💗 How the Nomination Process Works

1. Submit the Nomination Form
   Complete the form below with as much detail as possible about the nominee and their current journey.

2. Application Review
   After March 1st, all eligible applications will be carefully reviewed by the Sheryl’s BreastFest Executive Board along with past beneficiary recipients, who help us thoughtfully select our next honoree.

3. Selection of Beneficiary
   One nominee will be chosen as the 2026 Sheryl’s BreastFest beneficiary.

4. The selected nominee (or their family) will be contacted directly. All information shared in the application is kept confidential.

​

Nominees must:

• Be currently undergoing cancer treatment

• Be a resident of Polk County, Florida

• Be willing (if selected) to share their story with our community
